Please complete and sign this document. In addition, we require the following documents:

• Extract from the commercial register
• Statutes of the company
• Capital proof
• List of all persons authorized to sign
• Resolution for account opening

After receipt and review of the documents, we will immediately inform you about the opening of the account. You can now make the first transfer payments to the account.
